Polish Connection / S1 form
S1 form has replaced the former E-106/E-109 forms. It certifies the entitlement to healthcare in Norway guaranteeing the interested party and his family members access to necessary medical services in other countries of the European Economic Area (e.g. Poland). The Norwegian Health Economics Administration (HELFO) is responsible for issuing forms in Norway. It is the institution the application should be submitted with. Should the HELFO send the form to the applicant during the verification, it is necessary to deliver it to the appropriate department of the National Health Fund in his country. Sometimes it happens that the HELFO sends the form directly to the National Health Fund, which contacts the interested party.
The S1 form is issued to a person working in Norway and/or their family living in another EEA country.
